Description and Services
Description (Service) Supports and Services for youth 14-28 years of age who have an intellectual disability to plan and prepare for the transition from school to living and working in the community. Staff work in partnership with youth, families and schools to begin planning early, ensure the youth's goals and dreams are the focus of services provided in areas such as employment, leisure, education, and aspects of daily living. Assistance to access and utilize Ministry Funding, Developmental Services and ODSP is also available. Staff provide case management services, advocacy support and facilitate inclusion in the community. A variety of workshops and training are offered such as Cooking Up Fun, Social Connections, Home of My Own and Building Healthy Relationships.
