| Description (Service) | Supports for Adults living with prediabetes or diabetes. Education Clinics provide: *certified diabetes educators , registered dieticians and nurses who work in cooperation with physicians, pharmacists and family members * individual and group appointments *referrals and monthly telehealth sessions with specialists for advanced care Education on: * diabetes self-management * healthy living and eating * hyperglycemia (high blood sugar) * hypoglycemia (low blood sugar) * common emotions * physical activity * medications |
