Description (Service) Medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurses, nurse practitioner, dietitian, social worker, occupational therapist, and respiratory therapist 
 
Services include: 
* referral to specialty clinics 
* preventative health services 
* flu shot clinics 
* mental health services 
* dietary counselling 
* specialty group programs 
* sexual health services including family planning, pregnancy tests and sexually transmitted diseases 
* well baby visit program in the clinic at 18 months 
* postpartum depression
