Description and Services
Description (Service) Nonprofit group day program for fragile older adults living at home * provides Physical, Emotional, and Intellectual health, Capabilities, social and physical Environment, and Social self (PIECES) comprehensive assessment * programming includes physiotherapy, social therapy, cognitive therapy depending on needs of the individual * respite care and education for their caregivers
Hours Mon-Fri 9:30 am-3 pm
Areas Served Lanark ; Perth ; Smiths Falls
Eligibility Ages: 55 year(s) and up 
Participants: seniors 55 years and older who exhibit cognitive, social, and/or functional limitations that threaten their ability to remain in their home 
Volunteers: respectful, patient and dependable individuals with a sense of humor * must be willing to commit to the program for six months * must have a clear police records check
