Description (Service) | COVID-19 (20 Mar 2020) Call the Centre at 613-737-5115 before attending. Many of the programs and services are currently suspended or changed to phone appointments until further notice. Using a Health Links approach, registered nurses and community health workers visit vulnerable seniors in their homes to provide support with their care and link them to appropriate community resources to assist them with their various needs. * Program works in collaboration with health professionals and other agencies to reduce isolation and support high risk seniors living at home. |
