Description (Service) COVID19 (4 Jun 2020): Service remains available * appointments are being held virtually * group sessions are currently paused but may soon be held online.

Supports for people living with diabetes

Team includes a registered nurse and a dietician * in partnership with the client’s physician * initial evaluation, group session and individual follow-up * participants learn to better control their diabetes and help prevent health-related complications
Hours COVID19: Mon, Tue, Thu, Fri 9 am-4:30 pm * Wed 9 am-8 pm
Areas Served Cornwall ; Prescott and Russell United Counties ; Stormont, Dundas and Glengarry United Counties
Eligibility Ages: 18 year(s) and up 
Adults 18 years and older with diabetes and prediabetes
Application Self-referral * bring the Ontario Health Insurance Plan (OHIP) card
