Description (Service) | Coordinates all admissions into long-term care homes (including short-stay and respite care beds) and into complex continuing care (chronic care) facilities across Champlain * refers placement applications and assists in the coordination of admissions of long-term care facilities in other regions of Ontario Information regarding: retirement homes * community support services and programs |

Fees | Service ; Placement is free. The basic accommodations rates are set by the Ministry of Health and Long-Term Care and some subsidies are available. There is a cost for long-term or respite care in a long-term care facility bed and a cost for a complex continuing care hospital bed as well. |
