Description (Service) | COVID-19 (24 Mar 2020): Due to COVID19, PQCHC is closed for all but essential services until further notice. The ACTT is still available by phone at 613-820-4922 ext 3369. Assertive Community Treatment Team (ACTT): Voluntary program with a trans-disciplinary team which may include a psychiatrist, nurses, social workers, mental health workers, vocational rehabilitation worker, recreation therapist, occupational therapist, an addictions worker, and a peer support worker. Provides a wide range of comprehensive community-based services to clients on a year-round basis. Client-centered approach, includes family and significant other involvement with client consent, multidisciplinary treatment team, illness treatment and improved quality of life goals. Step Down Program: Designed for clients of the ACTT program who still require follow-up and support but not in as an intensive manner as provided by the ACTT. |

Eligibility | Ages: 16 year(s) - 75 year(s) Persons between the ages of 16 and 75 with persistent or chronic mental illness. This service is intended for those with the highest intensity of need due to complex, long-term, serious psychiatric issues. Eligibility for receiving ACTT service will be determined by a consultation with the client, ACTT staff, and a central intake committee. Individuals who think they might be eligible for service, or are interested in obtaining it, are encouraged to discuss it with their current service providers. |
