Description (Service) | Helps individuals better manage chronic pain by providing education on chronic pain and disability management * increasing activity level and helping people remain active even though they experience chronic pain * improving physical fitness and posture * improving emotional functioning The premise of the program is self-management, as opposed to attempting to cure or alleviate pain symptoms. Operates on an out-patient basis and typically lasts four weeks (one week of assessment/education and three weeks of treatment). The interdisciplinary team is comprised of professionals from nursing * physiatry * psychology * occupational therapy * physiotherapy * social work * vocational rehabilitation * recreational therapy * pharmacy |

Eligibility | Ages: 18 year(s) and up Individuals 18 years of age and older, referred by a physician, who have had continuous pain for three months or longer that has been thoroughly investigated and generally non-responsive to medical treatment, experience some or all of the following secondary conditions: physical deconditioning, difficulty coping with role changes, poor self-esteem, sleep disturbance, emotional distress, vocational uncertainty, and interpersonal difficulties, and are open to making lifestyle changes through a self-management approach. Individuals are not eligible if their primary pain symptoms are exclusive to headaches, gastro-intestinal problems, and urinary, pelvic, or gynecological difficulties, and/or they are seeking prescriptions, epidural blocks, or other medical treatments. |
