Description (Service) Passport/Individualized Funding for community participation and/or caregiver respite services and supports. 
* Assist people who receive Passport/Individualized funding to: Administer/Broker funds, Develop a Person-Directed Plan, Participate in recreational/educational programs and volunteer/employment opportunities, Complete activities of daily living
