Description (Service) Assertive Community Treatment Team for people with a dual diagnosis (mental health and developmental disability) 
Services: Treatment, rehabilitation and support * symptom management * advocacy * monitoring and assessment of medications * helps with hospitalization when necessary * long-term clinical relationship * tailored services to individual needs * meeting the basic needs and activities of daily life * helps with social environments and relationships * supports family relationships * helps with work and recreation * skills teaching * works with community and mental health services * behavioural support

Eligibility Ages: 16 year(s) and up 
Adults 16 years and older, who live in the service area, have a severe and persistent mental illness, have a developmental delay, are heavy users of psychiatric services and have complex treatment needs
