Description (Service) Local chapter of national organization providing the following services: Adult Day Programs (Perth, Carleton Place, Almonte and Smiths Falls) * family and caregiver support groups * individual support * Early Stage support * information and referral * educational workshops and seminars * lending library * Finding Your Way * MedicAlert® Safely Home® * overnight weekend respite program and Day Programs on Saturday and Sunday (in Smiths Falls) * Minds in Motion: two-hour physical and mental exercise program for the person with dementia and care partners * Brain Wave: education and discussion group for people in the early stages of dementia
