Description and Services
Description (Service) Provides residential care to individuals with a mental illness who no longer require hospitalization * administered by the Brockville Mental Health Centre, the Program is funded and residences are licensed by the Ontario Ministry of Health and Long-Term Care * each home is privately owned and operated 
Services include: Meals, laundry and 24-hour supervision on a short- or long-term basis * On-site medical care, via the house physician or family physician * Funding for personal care items, such as toiletries, clothing, eyeglasses and dental care * Monthly comfort allowance * Transportation * Social and recreational activities; and * Support from staff of the HSC home and HSC program case managers 
There are a total of 15 Homes for Special Care being administered by BMHC: 
* 8 homes in Ottawa-Carleton; 
* 2 homes in Leeds and Grenville; 
* 3 homes in Stormont, Dundas and Glengarry; and 
* 2 homes in Prescott and Russell.

Eligibility Ages: 18 year(s) and up 
To be considered for the Homes for Special Care Program, an individual must: 
* Be 18 years of age or older; 
* Have a mental illness that is currently stable; 
* Require 24-hour supervision; 
* Agree to the conditions of co-operative living; 
* Require assistance with self-care; 
* Be discharged from a specialty psychiatric unit or be registered as an outpatient 
Patients may not be placed by the Homes for Special Care Program if community service plans cannot ensure the safety of all residents in the home. This includes applicants who: 
* Have a recent history of uncontrolled violence and are clinically unstable; 
* Have a history of behaviour that poses a serious fire hazard, uncontrollable in an HSC setting; 
* Have a primary diagnosis of a developmental disability or; 
* Require 24-hour nursing care or a locked, secure setting
