Description and Services
Description (Service) Multi-disciplinary team of health professionals that provides assessment and treatment to individuals with established heart disease * prevention of cardiac and vascular (blood vessel) disease * assessment and treatment for: heart attack, angioplasty, by-pass surgery or testing suggesting developing atherosclerosis or heart disease, congestive heart failure, leg claudication, cardiomyopathy and those pre- and post-heart transplant * professional services for clients: individual nutritional and psychosocial counselling, exercise prescription and monitoring, and education about cardiac disease

Eligibility Anyone who has suffered from heart disease, angina, heart valve disease
Application Call to generate a referral * referral from family physician, nurse practitioner, cardiologist, cardiac surgeon required * bring a valid health card to every visit
Languages English ; Interpretive Services - available with 48 hours or more notice prior to appointment
Fees None - Costs covered by the Ontario Health Insurance Plan (OHIP) and other private health coverage
