Description (Service) Opiod Substitution Therapy * confidential * health clinic focused on those with opioid dependence * Methadone Maintenance Treatment (MMT) and Suboxone Substitution Therapy (SST)
Hours Mon, Wed, Thu 8 am-4 pm * Tue 7 am-5 pm * Fri 7:30 am-4 pm * Sat-Sun 9 am-12 noon
Areas Served Leeds and Grenville United Counties - and surrounding areas
Eligibility Individuals with an opioid dependence
Application Clients may self-refer by calling or visiting the clinic during regular clinic hours. Referrals also accepted. * Intake paperwork is provided as well as requisitions for lab tests. * Clients must provide valid Ontario Health Card upon initial visit.

Fees Service ; None ; In Clinic services are covered by the Ministry of Health with a valid Ontario Health Card. Clinic staff may offer assistance in updating/renewing Health Card for clients who require support.  
Note: There are fees for medication dispensing at the pharmacy that are covered by a valid Ontario Drug Benefits card (Ontario Works, ODSP). If you are working with no health insurance, clinic staff may assist you with applying for insurance coverage.
