DescriptionCommunity Care Access Centres (CCAC) provide one stop access to health and personal support services to help people of all ages live independently in their home and their communities. CCAC’s work with individuals and their families to assess specific needs and develop a plan of service. When living independently is no longer possible CCACs help investigate alternatives such as supportive residential options * retirement homes * long term care homes * convalescent * and respite care. They are the access point to Long-term Care Homes. CCAC’s connect all residents in the communities that they serve with the health and social services that they need through their Information and Referral Service www.CommunityCareInfo.ca. Care Connectors help people without family physicians to find a family health care provider.
