|Description||Program for kids 17 and under who do not have access to any form of dental coverage. Children who are eligible will have access to: check-ups * cleanings * fillings * x-rays. Services not covered: orthodontics * space maintainers * cosmetic dentistry * teeth whitening |
Local public health units across Ontario are leading the program in communities and are working with local partners such as Community Health Centres, primary care providers, dentists, dental hygienists, hospitals, schools and universities to deliver this program.
|Eligibility||Ages: 17 year(s) and under |
Children who are 17 years old or younger, with no dental insurance and who have a household Adjusted Family NET Income of $20,000 or less per year.

1. Annual Canada Child Tax Benefit and Ontario Child Benefit Notice OR Annual Goods and Services Tax Credit Entitlement Notice (most recent year).
2. Parent/Guardian - bring your valid Ontario Health Card (with photo)
If OHIP is not available, bring your Passport, OR Certificate of Canadian Citizenship (Citizenship Card) OR Permanent Resident Card, OR Canadian Immigration Identification Card, OR valid Ontario Driver's License.
3. Child - For each child bring their valid Ontario Health Card
If OHIP is not available, bring child's Passport, OR Certificate of Canadian Citizenship, OR Permanent Resident Card, OR Canadian Immigration Identification Card, OR Birth Certificate as issued by a Canadian provincial government OR valid Ontario Driver's License AND household mortgage, rental or lease agreement OR utility bill (to confirm address).
