DescriptionStroke prevention clinic for patients who have been diagnosed with a possible Transient Ischemic Attack (TIA) or a small stroke and who have been deemed stable by the referring physicians to have their diagnostic testing completed as an outpatient * Nurse practitioner in charge in consultation with a physician. 
 
Services: Rapid access to local and regional diagnostic services * Health assessment and follow-up * Medical treatment and appropriate referrals to specialty as needed * Patient/family education regarding the warning signs of a stroke, risk factor identification and lifestyle changes * Free access to a dietician * Possible linkages to other community programs to assist patients with their personal health management: Diabetes Clinic, Smoking Cessation, Cardiac Rehabilitation * Written report to the family physician after patient’s visit at the clinic to insure communication between health care providers
EligibilityPatients with suspected transient Ischemic Attack (TIA) or mild stroke history
