| Description | A client-centered, recovery-oriented mental health delivery service model. A team of mental health professionals, including a team psychiatrist provide community based treatment and support to persons with serious mental illness and significant functional impairment. The team works with the person to identify goals and assist them with their treatment and rehabilitation needs including housing, leisure, work and study. Psychiatric assessment * treatment and monitoring * individualized treatment, including medications * crisis intervention * skills teaching * assistance with housing * active vocational and recreational support * social work * nursing and occupational therapy * addictions expertise * Step Down Program: This is an intensive case-management program with five multidisciplinary staff and a psychiatrist. Referrals are received from five Ottawa based ACT teams. The Step Down team works with clients who no longer require the intensity of ACTT services but continue to require community support and follow-up. | |||

| Eligibility | Ages: 16 year(s) and up Age 16 and over with serious mental illnesses that are complex and result in significant functional impairment. * Persons with a high hospital service use – 50 inpatient hospital days in the past year or 150 over the past 3 years. Persons referred to ACTT often have difficulty accessing traditional services and may experience homelessness, involvement in the criminal justice system and a concurrent disorder (mental illness and problematic substance use). | |||
