DescriptionA program that can help individuals prevent diabetes or improve their diabetes control and help them live healthy with diabetes. Through education, individuals learn to adjust their lifestyle to control their diabetes. * hyperglycemia (high blood sugar) * hypoglycemia (low blood sugar) * Common feelings and emotions associated with diabetes * Healthy eating plans * Physical activity with diabetes * Staying healthy with Diabetes * Diabetes medications 
 
Five Hospital education Clinics: The team consists of certified diabetes educators which include registered dieticians and registered nurses. They work in cooperation with physicians, pharmacists and family members to deliver client-centered care. Individual and group appointments. For advanced care, they provide referrals and telehealth sessions with specialists. 
 
Outreach Sites: Provide screening assessments to determine an individual’s risk for developing diabetes & Provide both individual and group education and counselling services for clients diagnosed with diabetes or prediabetes. 
 
Renfrew County Pediatric Diabetes Education Program services can be accessed at Pembroke Regional Hospital directly or through Telehealth in partnership with the Children’s Hospital of Eastern Ontario (CHEO).
