DescriptionProgram helps people to manage their type 2 diabetes and pre-diabetes. 
 
The nurse and dietitian teams provide: group education * individual follow-up * insulin initiation * information on nutrition * exercise * glucose monitoring * stress and emotions * preventing complications * foot care and other self-care practices * centralized registration.

EligibilityAges: 18 year(s) and up 
 
Adults 18 years and over with type 2 diabetes or prediabetes.
ApplicationSelf-referral or a physician referral for insulin initiation.
