Hours	Mon-Wed 8:30 am-8:30 pm * Thu-Fri 8:30 am-4:30 pm
Description	Program helps people to manage their type 2 diabetes and pre-diabetes.    The nurse and dietitian teams provide: group education * individual follow-up * insulin initiation * information on nutrition * exercise * glucose monitoring * stress and emotions * preventing complications * foot care and other self-care practices * centralized registration.    Locations include: Centretown Community Health Centre * Pinecrest-Queensway Community Health Centre * Rideau, Osgoode CRC * South Nepean Community Health Centre * Western Ottawa Community Resource Centre * Orleans-Cumberland Community Resource Centre * Overbrooke-Forbes Community Resource Centre * South East Ottawa Community Health Centre
Eligibility	Ages: 18 year(s) and up    Adults 18 years and over with type 2 diabetes or prediabetes.
Application	Self-referral or a physician referral for insulin initiation.
