DescriptionOffers a Long-Term Care program to provide support to individuals of all ages. These services are intended to enable individuals to live independently. 
 
Transportation to medical appointments * friendly visiting * caregiver support * information * education * referrals to counselling agencies * security check/reassurance calls * advocacy
EligibilityIndividuals of all ages living with a chronic or debilitating condition.
ApplicationCall to apply * Referrals accepted from family members, caregivers, health professionals, community agencies or self-referral.
