DescriptionThe service is designed to "open doors" for persons with dual diagnosis in the mental health and developmental service sectors, as well as create new options for support. 
 
The service promotes a rehabilitative person-centered approach that responds to the breakdown of a person's support system. "Brokers" develop long-term solutions for people. The service promotes collaborative efforts that integrate partnerships among families, agencies, services, and ministries. 
 
Increase access to appropriate services and supports * facilitate training to developmental and mental health services professionals to increase awareness of specific needs of clients and their families * facilitate short and long-term planning to clients and their support networks using a Wraparound model as appropriate * monitor and assess service system gaps and report findings to relevant ministries * provide short-term direct outreach support on an as-needed basis
