DescriptionAdvocacy group to reduce the isolation of people with disabilities through several programs including: 
 
* The Everyday Champions program: Matches volunteer advocates to people with disabilities in one-on-one, long-term supportive relationships. Advocates provide practical and emotional support according to the needs of the person to whom they are matched. With this support, people are able to increase their ability to live independently, to participate in the community and/or to achieve personal goals. Group Home advocates provide in-home advocacy supports to residents of group homes. 
 
* The Chance for Choice program: Enhances the well-being of older adults with a disability who may be vulnerable, isolated or at risk. The older adult is matched to a volunteer in a long-term, supportive relationship. The volunteer provides practical and emotional support, and can be a social lifeline keeping the individual linked with other people and resources in the community. These connections enhance the resources available to the individual to make and realize choices on how they live their lives; they also reduce isolation and vulnerability to mistreatment, neglect or abuse. 
 
* The Lifetime Networks Ottawa (LNO) program: Facilitates the establishment and ensure the continuance of a loving and caring Personal Support Network in the lives of persons with a disability. Provides families with a trained community connector to help them build a circle of committed friends around the individual with a disability. This network actively supports the person at the centre in a variety of ways. LNO monitors the networks for the lifetime of the person, ensuring that the network is active and functioning as it should be. In addition, LNO offers workshops on topics of interest to families who have a son or daughter with a disability to help them plan a good life, both now and in the future. Also provides social activities that further reduce the isolation of our members and to provide an opportunity for families to network.
