| Description | Community health and resource centre. The Assertive Community Treatment Team (ACTT) provides services to people who have long-term mental health needs. Services include: Rehabilitation activities * treatment intervention * crisis intervention * individualized recovery planning * classes and groups Health Promotion services include: Community development * physical activity * prevention and management of chronic diseases * stress management * stop smoking programs * food security programs * special focus on seniors, youth, newcomers Counselling services include: Help in a crisis * mental health support * youth, individual and family counselling * substance abuse programs * help to find housing and jobs * stress reduction programs * groups with a special focus on women who have experienced abuse and youth who need support to express anger in healthy ways Medical clinic services include: Comprehensive health care * immunization programs * family planning * well-baby and child care * management of chronic and short-term health problems. Registered Nurses can provide health information and advice by telephone. Offers referral services to: Nutrition counselling by a registered Dietitian (groups and individual) * home, hospital and palliative visits * high school health clinics led by a Nurse Practitioner * foot care and chiropody * anonymous Human immunodeficiency virus (HIV) Testing * lung health programs * seasonal influenza and other immunization clinics. Also has an afternoon walk-in clinic for registered patients. | |||
