Hours	Mon-Tue, Thu-Fri 9 am-4:30 pm * Wed 1 pm-8 pm
Description	Primary health care, mental health care and community capacity building health promotion primarily for the Francophone community. English services available in Crysler and in Bourget.    Via its different programs and services, aims to improve health among its members and favours autonomy and responsibility of individuals.    * Community Health Program: improve health among members in the community    * Mental health Program: offers support services, evaluation, orientation, and individual, family and group therapy for people suffering from all kinds of problems * offers help in difficult times and with family problems    * Physical Health Program: offers consultation with medical, nursing and nutritional staff. Health promotion and health care services offered to the francophone members of the community. Complete medical examinations with follow-up appointments. Doctor's evaluation, electrocardiograms, expert laboratory services. Annual exams for babies, children and adults. Evaluation of eating and lifestyle habits. Screening, hospital visits, possible home visits.
