DescriptionDeliver one-on-one care to people over the age of 16 years in their own homes, schools or workplaces * helps with daily activities such as -- taking medication, personal grooming and hygiene, rising and retiring, dressing and undressing, bathing and washing, transferring, toileting, bowel/bladder services, range of motion exercises and essential communication tasks
EligibilityAges: 16 year(s) and up 
 
Open to those aged 16 years and older, able to direct own care, valid health card and living in Ontario, has a permanent physical disability and is not in receipt of insurance funding designated for attendant care services
ApplicationSelf/family/friend or doctor referral * contact the office for an application and assessment for program
FeesService ; None ; If funded hours are available there is no fee for this service * Where funded hours are not available there is a fee based option
