| Description | Provides one stop access to health and personal support services to help people of all ages live independently in their home and their communities * works with individuals and their families to assess specific needs and develop a plan of service * when living independently is no longer possible, helps investigate alternatives such as supportive residential options, retirement homes, long term care homes, convalescent and respite care * provides the access point to long-term Care Homes * connects all residents in the communities that they serve with the health and social services that they need through their Information and Referral service www.CommunityCareInfo.ca * Care Connectors help people without family physicians to find a family health care provider |
| Eligibility | No restrictions * eligibility is determined by a SECCAC Case Manager * to be eligible for in-home CCAC services, a person must have a valid Ontario Health Card, must have needs that can not be met on an outpatient basis, and a medical condition that can be adequately treated in the home |
| Application | By telephone (ask for Intake) or written request * CCAC staff are also available in hospitals across Ontario to help plan for patients' return home |
