DescriptionImproves quality of life for residents in the community by helping them to function independently in their own community through social and practical services and the provision of information and referral.  
 
Community Support Services (CSS) include: 
Caregiver Support Services such as: In-Home Respite (paid staff) * Education, Training & Information * Support & Counselling * Adult Day Service * Music & Memories Luncheon * Other CSS services include -- Essential Transportation * Friendly Visiting * Meals on Wheels * Foot Care Clinics * Client Intervention & Assessment * Telephone Reassurance * Congregate Dining (Meals On Wheels) * Home Help/Homemaking * Home Maintenance & Repair 
 
Primary Health Care Services include: 
Community Family Health Team (3 Brockville sites and 1 Gananoque site) * Mobile Primary Health Care Services * Primary Health Nursing (Wellness) Clinics * Value For Life * Lifeline * CPHC Seniors' Exercise Program * 10 Step In-Home Seniors' Exercise Program * Diabetes Education * In-Home Foot Care * Paternity Testing
