Description	Provides services to assist seniors and disabled adults to maintain full independent lives while living in their own homes and communities.     Community Support Services include: adult day program * Meals on Wheels * transportation to medical appointments * foot care * home assistance * information and education programs * friendly visiting * heavy home maintenance registry * diner's club * security/reassurance checks * recreational and social events
Eligibility	Ages: 18 year(s) and up    Services available to frail older adults, convalescent adults and adults with physical disabilities
