DescriptionCoordinates in-home services, such as nursing, physical therapy, occupational therapy and personal support, for people who are recovering from illness or injury, and for those who are living with chronic illness or disability 
 
Services help people avoid hospitalization, or return home quicker after being in hospital, so that they can recuperate in the comfort and privacy of their own homes * Also provides in-school services to children with disabilities, so that they can be full participants in school life * Contracts with private and not-for-profit agencies to provide these high quality services as effectively and efficiently as possible * When people are no longer able to manage at home, the CCAC helps them consider other housing options and coordinate admissions to long-term care homes
